Covid Vaccination Certificate
Please note that the practice does not supply proof of Covid Vaccination certificate.
If you have a smartphone the best way of accessing a Covid Vaccination certificate is to download the NHS APP. Once you have had registered with the NHS App you will be able to access a Covid Vaccination Certificate.
If you have had both Covid Vaccinations it will appear in the NHS APP two weeks after you have had the second vaccination.

The coronavirus vaccination programme continues. We are working with other practices in Slough to provide Covid vaccinations from two sites in Slough. These are Salt Hill leisure centre and Langley Health Centre. The NHS intention is that all adults over 18 will be vaccinated. If you have not been invited yet please be patient as we will shortly be inviting you.
Getting vaccinated only protects you from the virus, so you will still need to follow IPC and testing measures in your workplace, and follow general advice at work, at home and when you are out and about:
• Practice social distancing
• Wear a face mask
• Wash your hands carefully and frequently

AccuRX is now being used at the practice
The practice is beginning to use a system of individual text messages to patients called AccuRX. We will be able to text individual patients as regards their clinical care. If you do not want to receive text messages please contact reception so that you can opt out of this system. Please also contact reception if your mobile number has changed so that we have accurate records.
Our privacy notice explains why we collect information about you, how that information will be used, how we keep it safe and confidential and what your rights are in relation to this. Please see our privacy notice in the further information section.
If you provide us with your mobile number we may use this to send you appointment reminders or other health screening information. Please let us know if you do not wish to receive reminders on your mobile.
